During your initial visit, the Naturopathic Doctor (ND) will take a thorough case history, and may discuss the need for further investigative testing. The first visit with the ND is an information gathering session focused on understanding the patient physically and mentally to obtain the course of treatment best suited to the patient and their current complaints.
I understand that the ND will answer my questions to the best of his/her ability. I understand that the results are not guaranteed. I do not expect the ND to be able to anticipate and explain all risks and complications. I will rely on the ND to exercise the appropriate judgment relating to my best interests, based on the facts and findings then known.
I understand that charges are to be paid at the time of the visit unless specific arrangements were made prior to my scheduled appointment. Payment for all dispensary items is due at the time of receipt.
I intend this consent form to cover the entire course of treatment for my present condition. I understand that I am free to withdraw my consent and to discontinue care at any time.
